O: See surgery report for details. Placed IM-ESF tie in to repair fractured L TBT. Bruising, edema and skin abrasion present under the feathers, consistent with a crushing injury. Fracture site is comminuted with multiple butterfly fragments.
A: Repaired L TBT fracture. Lots of pieces of bone fragment present at the fracture site, bird may end of loosing a little bit of length in the limb. Leg very bruised/edematous from the trauma.
P: Start laser therapy on leg tom, monitor pin sites and sutures for discharge, dehiscence, monitor use of L leg and foot.

and intubated with 3.5 uncuffed ET tube. Placed bird in dorsal
recumbency and removed splint from L leg. Plucked and prepped
L TBT for surgery. Incised over the medial aspect of the leg
with a #15 blade. Dissected through muscle layers to the fracture
site. Muscle very edematous/bloody. Placed IM pin in the proximal
fragment, drove up out the flexed stifle. Blunted the distal
end of the IM pin, lined up the fracture ends and drove the pin
across the distal fragment and seated near the hock. Lots of
small bone fragments present at the fracture site. Too intra-op
x-rays to confirm placement, flushed wound site and closed muscle
in simple continuous pattern and the skin in a simple interrupted
pattern with 4-0 webcryl. Placed smooth cross pins in each fragment.
Bent the IM pin laterally around the leg and secured the cross
pins and IM pin to each other with epoxy. Bird was a little
slow to recover from anesthesia, BP seemed low, but eventually
recovered fully and uneventfully from anesthesia. |

Weight: 725.0 grams, BCS: 3.0 S: Finder reported bird falling into backyard and being brought in by dog. Arrived alert but unable to stand in transport box. Very alert throughout exam.
O: Medicated with butorphanol IM prior to beginning exam. Took radiographs. Placed splint on L leg. Gave fluids, oral and IM meds. Placed in fabric sling in crate in ICU.
------------- PHYSICAL EXAM FINDINGS ---------------- Mentation: BAR Temp (F): 103.0 Dehydration (%): 3 Heart rate: DNE Resp rate: DNE Neurologic: WNL Head/mouth/beak: Mouth tacky. Ears: NSF. Eyes: See ocular exam. Wings: NSF. Legs: L leg lax, abnormally rotated. Closed fracture of TBT found on palpation. Feet: Heavily reduced but slightly present use of L foot. Body: Small bruise mid-keel. Feather condition: Most tail feathers missing - suspect traumatic molt. Ectoparasites: None found. Radiographic findings: Fairly comminuted transverse fracture to L TBT. Initial treatment: 0.15cc butorphanol IM L of keel. Applied splint to L leg to stabilize TBT fx. 0.12cc Simbadol IM R of keel. 0.24cc meloxicam, 0.73cc TMS. 20cc LRS SQ in R inguinal.
A: Patient appears to be in otherwise fair condition. Fracture to left tibiotarsus is highly fragmented but a good candidate for surgical repair. Suspect trauma is a result of impact to the left side. Will need to be thoroughly checked over for puncture wounds from dog interaction. Heavily guarded prognosis due to necessity of orthopedic surgery.
P: rx meloxicam, Calcium slurry, pentoxifylline, TMS, Clavamox. rx Tramadol beginning 11/19 PM. 11/19 - fecal, blood, check stain uptake + IOP values. Check over for puncture wounds. 11/20 - vet check + TBT surgery. Monitor tightness of splint and adjust PRN.
